When you schedule a session online, we collect and store the following personal information:

  • Telephone Number

  • The state that you reside in
  • An intake form for each counselor you select to have a session with. These questions are provided to the counselor to ensure the best possible experience. This information is not shared with anyone other than your counselor.

We also collect and store the following technical information when you visit our site:

  • IP Address
  • Browser Type
  • Site Usage

We use the IP address, demographic and user preference information we collect from you to improve your user experience, improve the overall functionality of our website and contact you regarding your account or transaction.

From time to time, we may share your personal information with Elevate Therapy company representatives or agents, in order to provide services related to the maintenance, operation and marketing of the website. A service provider’s use of such information is restricted by confidentiality agreements between Elevate Therapy and such provider. Additionally, in special circumstances, your information may be shared with others in order to voluntarily comply with applicable laws, cooperate with law enforcement and protect the interests or safety of or other visitors to this site. In order to keep our customers informed about newsletters and special offers from Elevate Therapy and/or our partners, we may send such information via email, or telephone, based on customer contact information.
