What is the intake form and why do you need it?

Once you’ve booked your first appointment, you’ll be asked to fill out an intake form, this is exactly what you’d expect to receive at the beginning of your first in-person visit with a therapist – we just take away the clipboard!

Filling out your intake form is an important part of the therapy process. The intake form helps your therapist get a little background information on you and your current situation so that you can start your session off on the right foot.
